EXAMPLE:

Claiming a basic day penalty for being ran-around per Rule 66. On ___(date)     at      (time)    hrs. was first out on the   (circ-7)   (board id)     . CMS filled a vacancy on the (circ-7)   (board id)   (job id) , on duty for  ((time- same time as above first out time)    hrs. improperly calling   (hostler's initials and last name)   for the job. I was first out and available and should have been called for the vacancy.
