Arrived at BLAKESLEE Junction at hrs. Disembarked (train symbol) & boarded crew van at (time) hrs. Arrived back at the final terminal of (Circ-7) at (time) hrs. Claim 130 mile penalty in addition to all overtime and other earnings per Rule 16.
 
RECEIVING TRAIN AT BLAKESLEE JUNCTION EXAMPLE:
Was called out of away from home terminal of Centralia for the (train symbol) on (date) at (Circ 7) at (time) hours. At (time) hrs., upon reporting for duty, was ordered by (manager name & title) to depart on-duty terminal of Centralia in crew van and pick up train beyond terminal. Departed (Centralia switching limits Milepost) at (time) hrs. in crew van. Arrived at (train symbol) at milepost (BLAKESLEE Junction) at (time) hrs. Took charge of train and departed at (time) hrs., arriving back at milepost (Centralia switching limits) at (time) hrs. Then departed Centralia switching limits (milepost #) on regular assigned run toward OP500 at (time) hrs. Per Northwest District Rule 16, entitled to 130 mile basic day for doubling out of terminal.
